CSHM Awards of Excellence - Call for Nominations!

The Canadian Society of Hospital Medicine created its Awards of Excellence Program to honour our members whose contributions to the hospital medicine movement deserve acknowledgement and respect. Often the greatest contributions to patient care and the Hospital Medicine movement are not compensated. Please take the time to acknowledge the great work done by your colleagues and nominate them for an award to be presented among their peers. Eligible candidates are physicians practicing primarily Hospital Medicine in Canada.

There are three categories open for nominations:

• Clinical Excellence
• Outstanding Service
• Quality Improvement and Patient Safety

The awards will be presented at the 16th Annual CSHM Canadian Hospital Medicine Conference. This year the conference will be held in Whistler, British Columbia, from Thursday, October 25 to Saturday, October 27, 2016. The recipients will be awarded complimentary registration for the Conference and will be honoured at the evening reception on Friday, October 26, 2018. (Attendance at the awards ceremony is mandatory).

Clinical Excellence

Presented in recognition of exemplary clinical service in Hospital Medicine.
While all of the following may not apply to each candidate, the Awards Committee evaluates each candidate using the following criteria:

• Outstanding contributions to patient care
• Advancement of clinical knowledge
• Professionalism
• Leadership in medicine
• Excellence in practice management
• Subjective consideration

Outstanding Service 

Presented in recognition of exceptional service to the discipline of Hospital Medicine.
While all of the following may not apply to each candidate, the Awards Committee evaluates each candidate using the following criteria:

• Organizational activities
• Innovations in hospital or medical groups
• Leadership in organized medicine or the community
• Contributions in public policy
• Subjective considerations

Quality Improvement and Patient Safety

Presented in recognition of exemplary interprofessional quality improvement initiatives lead by or with significant contribution from hospitalists. The award is intended to highlight the role played by hospitalists in developing and implementing successful multi-disciplinary interventions that improve patient care & safety.
While all of the following may not apply to each candidate, the Awards Committee plans to evaluate each candidate using the following criteria:

• Broad-based participation and team approach
• Scope & magnitude
• Measurability
• Transferability & development of portable implementation guidelines or tools
• Role of hospitalists in planning & implementation
